First decide what you exactly need, whether its OT or Attested mark lists for your higher studies/immigration. Some asks for OT while some just need attested mark lists in a sealed cover. Fee and procedures for these two are little different. And if you are in confusion an go for the wrong one the amount paid at university is not refundable.
1. OT : contains separate sheets for each semester detailing the marks scored and the intention for giving the transcript. It doesn't contain any photocopies of your mark lists.
Fee : 2000/-
By default it comes in a sealed envelope to maintain the authenticity.
2. Attested mark sheets. You need to carry the photocopies of your marks sheets with you. They don't take photocopies for you.
Fee : semester mark list 300/- for each
degree certificate 400/-
Altogether for the entire attestation it will cost ( 7x300 + 400 )
By default the attested copies are not enveloped. If you want it in a sealed cover as OT is , you need to mention that.
The university either can send it directly to your institution or it can be handed over to you/ any person you authorize.
Steps :
1. Write a request letter heading to 'Controller of Examination, CUSAT' . Let me give a sample here.
From,
< name >,
< full address>,
< contact number ( must )>
To,
The controller of examination,
Cochin University of Science and technology,
< address >
< your college , year of passing out >
< your intention for writing the letter ( higher studies / immigration etc ) include university/ country name>
< be specific what you need ( OT / Attestation ) >
< tell how you need it ( in a sealed cover or just paper in hand ), ( mainly needed in case you need the attested copies in a sealed cover. ) >
< if you want to send it to any address specify it fully and clearly >
< if you want it in hand specify that also >
< if you are authorizing any other person to collect it mention his/her name and contact details >
< if you want any other documents to be filled by them and included within the sealed cover , mention the documents need and what all things to be taken care for that >
< done >
2. Carry any ID card with you in order to enter into university.
3. Carry photocopies( if going for attestation ), request letter
4. Admin block, 2nd floor , right end -> controller of examination.
5. Meet P.A of controller and give the letter.
6. Get the OT request form and fill it ( it can be downloaded from cusat website -> downloads )
7. Go to ground floor ( cash section ), pay amount, get receipt ( you will get 2 copies attach original with request letter and keep the copy with you )
8. Go to controller room and get seal on request letter from the secretory.
9. got to 'G' section ( there will be section of representing your college ) G is for MEC, Kalloopara etc... Give the Letter, cash receipt , photocopies ( if for attestation ), other forms.
10. Get the contact number from them to follow up.
Note :
1. Photocopies should be clear. Entire mark sheets should be visible in the photocopies. There should be some space left at the bottom for them to sign and put seal.
2. OT take average to 2 weeks and Attestation take average of 1 week.
3. If you are opting to send it directly to your institution they will send it only via normal post. Its a little risk. Better you go/authorize somebody to collect and send so that you can track it.
All the best.
